1. One valid ID (any of the following, mamili sa mga sumusunod) |
➔ Current employees ID with the signature of the employer or authorized representative |
➔ Postal ID |
➔ Students ID or library card, signed by the school authority; |
➔ Senior Citizens ID |
➔ Driver’s license |
➔ NBI/PNP clearance |
➔ Passport |
➔ SSS/GSIS ID |
➔ Integrated Bar of the Philippine (IBP) ID |
➔ License issued by the Professional Regulatory Commission (PRC) |
2. Number of copies being requested (Bilang ng kopya na hinihingi) |
3. Name (maiden name if married female, apelyido noong dalaga pa) |
4. Gender (Kasarian - lalaki o babae) |
5. Place of birth (Lugar ng Kapanganakan) |
6. Date of birth (Petsa ng Kapanganakan) |
7. Father’s name (Kumpletong Pangalan ng Tatay) |
8. Mother’s maiden name (Kumpletong Pangalan ng Nanay) |
9. Date of registration, if the birth certificate was registered late; and (Petsa ng pagrehistro, kung ang Birth Certificate ay nahuling irehistro) |
10. Purpose of your request (Layunin ng paghingi) |
11. For representatives: relationship to the owner of the birth certificate (Para sa mga proxy, o kinatawan ng humihingi - ilagay ang relasyon ninyo sa may-ari ng Birth Certificate) |
12. Fee: P330 per copy (Magbayad ng 330 pesos) |
