  • BS IN GEOLOGY
  • BS IN GEOLOGICAL ENGINEERING
  • MS IN GEOLOGY
  • NSO / PSA Birth Certificate
  • NSO / PSA Marriage Contract (for married female applicants)
  • Transcript of Records with scanned picture and Remarks "For Board Examination Purposes"
  • (First Timer & Repeater)
  • Payment:
  • P900.00
  • *A citizen of the Philippines or a foreign citizen whose country has reciprocity agreement with the Philippines.
