  • DOCTOR OF OPTOMETRY (SIX-YEAR COURSE)
  • NSO / PSA Birth Certificate
  • NSO / PSA Marriage Contract (for married female applicants)
  • Transcript of Records with scanned picture and Remarks "For Board Examination Purposes"
  • Payment:
  • P1,300.00 - complete (written & practical)
  • P900.00 - Written Only
  • P400.00 - Practical Only
  • *After three (3) failures, REFRESHER course
